An upstream fiber cut in the Southwestern US is causing packet loss for certain paths through one of our bandwidth providers. As a mitigation, we have adjusted routing at our Phoenix facility to prefer an alternate upstream provider where possible. We are continuing to monitor network performance following this change.

### Root Cause

The issue was traced to a fiber cut in the Southwestern United States affecting Cogent, one of the upstream networks through which Internet traffic may reach our Phoenix facility. This resulted in packet loss along certain routes and intermittent connectivity for customers whose traffic traversed those paths.

Our Phoenix network maintains redundant upstream connectivity through multiple providers. Because Internet traffic can traverse multiple independent networks before reaching our infrastructure, the impact varied depending on the route taken.

### Mitigation

While Cogent worked to repair the fiber issue, we adjusted routing preferences at our Phoenix facility to favor an alternate upstream provider. This was a temporary traffic-engineering measure to reduce use of affected external paths and did not require changes to address an issue within our datacenter network or infrastructure.

We continued monitoring network performance and routing behavior while repairs were completed.

### Resolution

Cogent subsequently completed repairs to the affected fiber infrastructure, and network performance stabilized.

After confirming normal performance across the affected paths, we removed the temporary routing preference and returned the Phoenix network to its standard redundant routing configuration. Monitoring following the change confirmed normal network performance.

### Conclusion

The upstream fiber issue was resolved by Cogent, and network performance returned to normal. We subsequently restored our standard routing preferences and confirmed stable network performance through continued monitoring.

  <title>Phoenix Data Center Power Maintenace</title>

Following successful commissioning of a new UPS unit, the building team will place one of the UPS systems into Maintenance Bypass.

During Maintenance Bypass, power will be supplied directly from utility service, and UPS protection will not be available during that period. No outage is anticipated. Once the replacement is complete, the building&#039;s team will perform operational tests to verify proper system functionality. Assuming all tests pass, the system will be returned to normal operation.

There will be no service interruption expected. However, during the bypass window, your server will not have UPS protection in the event of a utility power event.

  <title>KC1 Power Outage</title>

    Aug 1, 21:00:10 GMT+0 - Monitoring - A brief utility power outage occurred this afternoon from 3:59 PM CDT to 4:02 PM CDT. Utility power has been restored, and the facility is operating on normal utility power.

Our engineers are onsite investigating the cause of the outage and are working to restore any servers or services that did not automatically recover following power restoration.

A Root Cause Analysis (RCA) will be provided within 24–48 hours. Aug 3, 19:00:00 GMT+0 - Resolved - On August 1, the KC1 data center experienced a brief drop in utility voltage lasting approximately one second. While utility power was not completely interrupted, the incoming voltage fell below its normal operating level.  
  
The facility&#039;s UPS systems responded to the event as designed, using battery power to protect the equipment during the low-voltage condition. Because the event was extremely brief, the facility generators were not required to start. One UPS continued operating on battery power until its battery reserve was depleted, **resulting in up to approximately three minutes of service interruption for customers connected to that UPS** before the system automatically transitioned to bypass mode. Once in bypass, utility power continued to supply the affected equipment, but without the UPS providing its normal power conditioning and battery protection.  
  
On Monday, August 3 at approximately 2:00 PM CDT, our Engineering team successfully returned the affected UPS from bypass mode to its normal inverter (double-conversion) operating mode, restoring full power conditioning and battery protection.  
  
Our Engineering team is evaluating the configuration and settings of the facility&#039;s Automatic Transfer Switch (ATS) to determine whether adjustments can be made to better respond to short-duration utility voltage disturbances. As part of this review, Engineering may adjust the transfer thresholds so that similar events are more likely to initiate generator startup, reducing the likelihood of the UPS remaining in bypass mode following a brief utility disturbance.

  <title>Network Disruptions</title>

    Jul 13, 19:00:00 GMT+0 - Monitoring - Connectivity has remained stable for an extended period following the restoration efforts, and we do not anticipate any additional disruptions at this time. We will continue to closely monitor the affected upstream providers and provide further updates if there are any changes or once the incident has been fully resolved. Jul 15, 15:55:10 GMT+0 - Resolved - On July 13, 2026, customers hosted in our Phoenix environment experienced a complete loss of Internet connectivity after all upstream providers serving the environment became unavailable simultaneously.

The outage was caused by a power event affecting networking equipment operated by our upstream providers during maintenance on infrastructure supporting their services. The providers advised that the maintenance resulted in an unexpected loss of power to their equipment and that expected battery backup systems did not perform as intended, resulting in a complete loss of upstream connectivity.

Because all upstream providers were impacted at the same time, external connectivity to the Phoenix environment was unavailable until provider equipment was restored. Patmos infrastructure remained fully operational throughout the incident.

Patmos engineers immediately engaged the affected providers and monitored restoration efforts until all upstream connectivity was restored and stability was confirmed.

As follow-up, Patmos has requested formal RCAs from the affected providers and will review the findings for any applicable operational improvements.

  On July 13, 2026, customers hosted in our Phoenix environment experienced a complete loss of Internet connectivity after all upstream providers serving the environment became unavailable simultaneously.

The outage was caused by a power event affecting networking equipment operated by our upstream providers during maintenance on infrastructure supporting their services. The providers advised that the maintenance resulted in an unexpected loss of power to their equipment and that expected battery backup systems did not perform as intended, resulting in a complete loss of upstream connectivity.

Because all upstream providers were impacted at the same time, external connectivity to the Phoenix environment was unavailable until provider equipment was restored. Patmos infrastructure remained fully operational throughout the incident.

Patmos engineers immediately engaged the affected providers and monitored restoration efforts until all upstream connectivity was restored and stability was confirmed.

As follow-up, Patmos has requested formal RCAs from the affected providers and will review the findings for any applicable operational improvements.

On 11/18/26 at 11:05 PM CST, the KC1 datacenter experienced a loss of utility power due to an upstream outage. UPS systems immediately carried the full datacenter load. Generator startup was initiated but did not complete before UPS battery capacity was exhausted, resulting in a facility-wide power loss. Utility power was restored at 11:43 PM CST.  
  
The generators successfully started and operated during routine testing and annual maintenance the prior week. During that maintenance, a cold-weather component (block heater) was identified for replacement, and repairs were already in progress. At the time of the outage, extreme cold temperatures and high winds prevented the generators from reaching operating conditions quickly enough to assume the load.  
  
Because the outage resulted in a hard shutdown of datacenter systems, additional time was required after power restoration to safely bring servers and supporting services back online, validate system integrity, and address any startup issues.  
  
Block heater replacement has been expedited, cold-start performance is being re-verified, and additional winter readiness checks are being implemented. Jan 19, 05:28:00 GMT+0 - Monitoring - KC1 experienced a power outage due to a loss of utility power beginning at 11:05:19 PM CST. UPS systems carried the load until approximately 11:28 PM CST, at which time the UPS failed and on-site generators did not start as expected.

Replacement parts have been expedited and will arrive Friday (November 21). Technicians are scheduled to be onsite at 6:00 PM MST Friday to complete repairs. We expect the UPS to be fully restored by late Friday evening or early Saturday morning.

On the evening of 11/19 at roughly 5:00 PM MST, we began receiving alerts as a building power changeover unexpectedly caused one of our UPS units to fall into bypass. A battery issue inside the UPS led to a sudden loss of power for the servers connected to it, resulting in hard shutdowns. Our team immediately moved into emergency response, including flying in additional technicians to help expedite service restoration.  
  
By 7:00 PM MST 11/21, UPS functionality had been restored, but the power event affected each server differently, which meant recovery required individualized attention rather than a single fix for all systems. Progress continued steadily through the night; by the morning of 11/22, most servers were back online, with only a small number still offline. By the morning of 11/24, the vast majority of servers had been restored, with only a few systems still undergoing more extensive hardware repairs.  
  
We are reviewing generator-transfer procedures and continuing routine maintenance and checks on the UPS and broader datacenter infrastructure to ensure safeguards are confirmed, strengthened, and any potential future issues are proactively detected. We are also updating our internal response playbooks based on lessons learned from this event.

  Root Cause Analysis  
  
On the evening of 11/19 at roughly 5:00 PM MST, we began receiving alerts as a building power changeover unexpectedly caused one of our UPS units to fall into bypass. A battery issue inside the UPS led to a sudden loss of power for the servers connected to it, resulting in hard shutdowns. Our team immediately moved into emergency response, including flying in additional technicians to help expedite service restoration.  
  
By 7:00 PM MST 11/21, UPS functionality had been restored, but the power event affected each server differently, which meant recovery required individualized attention rather than a single fix for all systems. Progress continued steadily through the night; by the morning of 11/22, most servers were back online, with only a small number still offline. By the morning of 11/24, the vast majority of servers had been restored, with only a few systems still undergoing more extensive hardware repairs.  
  
We are reviewing generator-transfer procedures and continuing routine maintenance and checks on the UPS and broader datacenter infrastructure to ensure safeguards are confirmed, strengthened, and any potential future issues are proactively detected. We are also updating our internal response playbooks based on lessons learned from this event..&lt;/p&gt;

A network service disruption affecting clients on a shared VLAN occurred between September 3-6, 2025, caused by MAC address table corruption within the datacenter leaf switch fabric. Root cause investigation definitively identified that the firewall leaf switch cluster was not properly clearing MAC address entries, propagating incorrect records to other leaf switches and disrupting connectivity. Initial detection occurred September 3 at 16:00 UTC through monitoring and client reports. Temporary mitigation via MAC table clearing provided intermittent relief, but the issue persisted until September 6 when all leaf switch clusters were systematically reloaded to rebuild the fabric. Full resolution was achieved by 18:00 UTC with extended monitoring confirming complete stability by 22:00 UTC. The root cause has been eliminated through the fabric rebuild, and additional preventive measures including migrating a subset of servers off the shared VLAN have been implemented to ensure this specific issue cannot recur.

  Root Cause Analysis  
A network service disruption affecting clients on a shared VLAN occurred between September 3-6, 2025, caused by MAC address table corruption within the datacenter leaf switch fabric. Root cause investigation definitively identified that the firewall leaf switch cluster was not properly clearing MAC address entries, propagating incorrect records to other leaf switches and disrupting connectivity. Initial detection occurred September 3 at 16:00 UTC through monitoring and client reports. Temporary mitigation via MAC table clearing provided intermittent relief, but the issue persisted until September 6 when all leaf switch clusters were systematically reloaded to rebuild the fabric. Full resolution was achieved by 18:00 UTC with extended monitoring confirming complete stability by 22:00 UTC. The root cause has been eliminated through the fabric rebuild, and additional preventive measures including migrating a subset of servers off the shared VLAN have been implemented to ensure this specific issue cannot recur..&lt;/p&gt;

Service was fully restored at 12:30 AM today when the fiber came back online. All affected routes have stabilized, and no further impact is expected.

  Power has remained stable throughout the day with no further incidents and services have been restored to impacted customers hours ago.

Our teams are continuing to work with the vendors responsible for maintaining our power delivery protection systems to further investigate that caused the incident today. They will be assessing the health of the power delivery protection systems that support our customers and applying any repairs as needed.
